STUDENT OF THE MONTH NOMINATION FORM

If you would like to nominate one of your students for Student of the Month, please just fill out the form below! Students can be nominated for any reason that you see fit, such as the following:

Made great improvement in some specific area
Competed/performed in a competition, recital, audition or other performance activity outside of EMA
Added a large piece of music to their completed repertoire
Completed an exceedingly large amount of practice hours
Overcame a personal playing obstacle they had been struggling with
Composed a piece of music
Consistency over a long period of time
Excelled above and beyond in a specific area

Please feel free to nominate one student, per location you teach in, per month. Please note that all nomination write-ups are shared with the students and parents regardless of whether they win, so please keep that in mind while writing your comments about the student. So choose your words carefully! The Student of the Month will be chosen by a random draw each month and awarded with a certificate, a trophy, and a spot in our Student of the Month photo display.

Nomination Write-Up

Please write a paragraph or two about why you feel this student deserves to be named Student of the Month. Describe their accomplishments in detail. If your student wins, this write-up will be shared with our entire school via our newsletter and waiting room TV, so avoid using statements about them being your favorite or best student. Please note that this write-up will be shared with your student and their parents regardless of whether they win or not! So choose your words carefully!
